NHS funded intermediate care

If the person you care for has been discharged from hospital they may be able to get free care for up to 6 weeks. This gives them time to get better when they no longer need to be in hospital. They could get this support at home or at a residential care home.

The hospital social workers will arrange NHS funded intermediate care before the person you care for leaves hospital. It isn’t means tested so it doesn’t matter what their income is, or if they have savings.

The person you care for will be assessed to work out what they need to get better. This could be paid support workers (sometimes called care workers) or support from other health professionals. For further information: Carers Trust
